2024 FOUNDATION ENROLMENTS
Thank you for your interest in enrolling at our school. To ensure that every family has the appropriate opportunity to enrol their child, all Ballarat Catholic Primary Schools collaborate throughout the process of Foundation (Prep) Enrolment. Information collected through the enrolment process is collated on a common database, which is shared by the Catholic Primary Schools in Ballarat.
The application process for 2024 Foundation (Prep) Enrolments:
1. Register for a school tour by clicking here.
2. Register your interest in enrolling your child at a Catholic school in the City of Ballarat by completing an online “City of Ballarat Expression of Interest for Enrolment” form found here. At this stage, you will be able to nominate St Patrick's Parish Primary School as your first preference.
While completing the Expression of Interest form you will be required to indicate your first, second and third preference schools and upload your child’s birth certificate and baptism certificate (if applicable).
Enrolment for 2024 Foundation (Prep) students is now open.
Completing this form does not guarantee that an enrolment offer will be made.
At St Patrick's Parish Primary School, a school tour is mandatory for all new families.

Thursday 25 May, 2023 Expression of Interest forms close
Friday 2 June, 2023 at 12.00pm First round placement offers sent (This step will consider siblings and children who are baptised Catholic within the local parish of the school)
Friday 9 June, 2023 at 4.00pm First round Acceptance closes
Wednesday 14 June, 2023 at 10.00am School principals meet to determine allocation of placements
Friday 16 June, 2023 at 12.00pm Second round placement offers sent and list of schools with vacancies published and sent
Friday 23 June, 2023 at 4.00pm Written acceptance of enrolment required from parents
Each Catholic Primary School will make offers of enrolment in line with the Diocese of Ballarat Enrolment Policy which takes into consideration a variety of factors.

IMMUNISATION HISTORY STATEMENTS
When enrolling a child in primary school, parents must provide the child’s Immunisation History Statement from the Australian Immunisation Register. This shows whether a child has been immunised against diphtheria, tetanus, polio, measles, mumps, rubella, whooping cough and haemophilus influenza type B. Failure to provide the Immunisation History Statement could delay your child’s enrolment to St. Patrick’s Primary School. AGE
Only children who will be five, on or before the 30th April of the year they enter Foundation will be accepted. |

ENROLMENT ENQUIRIES - YEAR LEVELS OTHER THAN FOUNDATION

TRANSFER FROM ANOTHER SCHOOL
Once a place is determined to be available and enrolment forms have been completed a transfer form is required for any children transferring from another school. Copies of reports would also be appreciated to help the teacher assess the child and help the child settle in to school life. Immunisation History Statements from the Australian Immunisation Register must also be provided when children are enrolled from other schools. When the child is transferring to another school, parents will be issued with a transfer notice to be taken with them. Parents may also be asked to complete a ‘Continuity of Care’ transition form for their child’s new school.
